Are you an individual with a developmental disability, a family member of someone with a developmental disability, or a service provider for people with developmental disabilities? My Home My Community (MHMC) has launched three surveys to get a better idea of what it takes to give people with developmental disabilities more choice in the kind of housing they live in. Please help us by taking a confidential survey.

Please note that developmental disability is used in this survey interchangeably with intellectual disability, and refers to people who identify or were identified before adulthood as having a developmental or intellectual disability.

Survey for Member Families

We need to understand the kinds of housing situations individuals with developmental disabilities are currently living in. We also need to know what kinds of housing situations they would like to live in. We are looking for information about what resources are already available that could help create new housing options for this group. We have created this survey to get more information on these questions. If you have a family member with a developmental disability who is 15 years or older, your answers will help us identify new pathways to inclusive and affordable housing for this group.

Survey for Service Providers

Part of this initiative involves identifying and studying existing assets in the community that might be used to create new suitable affordable housing options for this group. This survey aims to collect some of this asset inventory information. Your answers will be an important contribution to this effort and will help identify new pathways to housing for individuals with a developmental disability.
